I would say that the short stick d-middies in the MDIA don't get respect but in the ever slowing game of DI, those guys are highly respected. Coaches will call for a horn almost every chance they get to put in one of those guys. Also as that 5th man during man-down those guys are definitely sought after when recruiting comes around.
As for the most thankless job in sports.... I think that refs take the top.
