What is your e-mail address? I have looked, most likely in the wrong spot for it. I have mailing address and fax number.

You should have received two sets of forms. Also you should receive two sets in the mail.

Thanks.

Found it, just had to maximize the screen settings. The bottom part of the screen was covered up. Some one has been playing on my computer.

We Emailed ours so I don't know what the problem with that is about? But I did not have the address for the Physical copy until Kevin sent it today. I lost some important E-mail on my Desktop this past month and I believe in that was some SELC stuff. At anyrate I will get this hard copy out in the mail ASAP for Liberty.

Yeah, I never got an e-mail. To be safe, you might want to fax the original as well. My fax # is 978-374-8895.

I want to know what is bad about the old SECL form. It has the same information that is on the printed one. Both require the a signature and registrar seal. You have name of student, ID #. Can not the old form be used this year.

To get my registrar to sign another document right now is next to impossible. He just started doing graduation audits.

I know from an SVU point of view that you have a form with all the students signing. Then you have a form signed and verified by the registrar.

The problem with the old form is that it is not accessable on the internet. The main benefit of having this information on the internet is that teams can (and should) print out their opponent's eligible player list to bring to a game. You can't do that with the old forms.

As for the forms I received from SVU, it is not clear enough which players are being certified. For one, the registrar's signature is not on the same page as the players names. Second, it is not certified correctly since the registrar did not stamp it (the directions say that if there is no stamp, that they must include acknowledgement on official letterhead of Registrar's Office).

I sent you the hard copy of the form from the internet and it does have the registrars seal and signature. When I print out the form it is split over two pages. All the players that signed the old SELC are the same ones on the new USLIA form. Can you let me know if you have received it in the mail. They were sent last week.

You're right, the stamp is there. Those stamps can be hard to find sometimes.

And you're right about it splitting onto two pages because of the length. That's the issue, though. With the signatures on one page, the names the registrar saw on another, and the certification itself on another, it just doesn't work.

How exactly do you do this, I am not seen a way how. Maybe I am missing something here.

Ok, this is what I am going to do. As soon as I can of course. Print out another USLIA form. Have the registrar sign and stamp the page where the names are. But what about teams that have more players, their forms will a lot longer. Are their registrars going to have to sign and stamp all sheets. Because if the signature and stamp are on different pages then the list of students names that will be a problem. If I read you correctly.

No, it's only a problem since there were two set of sheets. What made that a problem was the fact that the registrar's signature was on a completely different sheet. What I actually need from the team is a form printed from the website, signed by all the players, and then stamped on the last page by the registrar.
