This topic will be used to keep teams updated regarding the Division B eligibility forms I have received. If you have any questions about the process, feel free to e-mail me or PM me. Remember, no hard copies need to be sent to me. It is only required that you fax the form to me. Your team should keep a copy of the original and the original should be sent to your conference. When faxing, make sure the team's main contact's phone number is included ON THE FORM. My fax number is 978-374-8895. You may fax at any hour you please.
Albertson: certified between 2/4 and 2/11, faxed before 2/18
Central Washington: certified between 2/4 and 2/11, faxed before 2/18
Lewis & Clark: certified between 1/29 and 2/5, faxed before 2/12
Linfield: certified between 2/5 and 2/12, faxed before 2/19
Montana: certified between 2/11 and 2/18, faxed before 2/25
Pacific Lutheran: certified between 1/22 and 1/29, faxed before 2/5
Puget Sound: certified between 1/21 and 1/28, faxed before 2/4
Southern Oregon: certified between 2/11 and 2/18, faxed before 2/25
Western Oregon: certified between 1/28 and 2/4, faxed before 2/11
Western Washington: ALL SET
Whitman: certified between 2/11 and 2/18, faxed before 2/25
Williamette: certified between 2/4 and 2/11, faxed before 2/18

Now that we have jumped through the eligibility hoops, let me put this out there. Walking into the registrar's office and telling them they have a week to fill out a form that no other sport or club on campus requires has definitely caused tension. However, doing so in the first week of the term has officially made me public enemy number one. I realize that if I adjust my schedule I can avoid the first week, but it seems like this process is even more stringent that the NCAA process. Granted they have rep on campus who handles these issues, every year it seems to be a little tighter. So, if possible, could we confirm the dates of the next eligibility check window? I am pretty sure we will go through this just before conference playoffs, and then nationals if qualified. Is this correct, and are there before and after deadline dates that will go that?

According to the USL MDIA checklist - a 2nd eligibility form is due to the Conferences (only) for archiving after the drop/add date for classes or after the beginning of Spring term for non semester schools.
You are correct in that a post season eligibility form is to be faxed to the eligibility chair 14-7 days prior to conference playoffs
