Bob Duggan can clarify the procedure, but the officials should automatically check all the field setup rules that are in the rule book. If one of the waivable rules is not in compliance, they should check with the home coach to see if it can be changed. If not, they should check with the visiting coach to see if he agrees to waive it. If he doesn't (which would be pretty bush-league, but perfectly within his rights), then it's a 3 minute plus a 30 second, so the home team would start down 2 men.
We have this issue in Oosterbaan, and we make our limitations clear to visiting teams when we schedule them, and have them sign a contract acknowledging the changes.
Only the rules listed in the rule book as waivable can be waived! Most rules are not negotiable. Things like legal goals must be enforced, even if the visiting coach says he's willing to waive them. It's not a choice he has. Non-compliant uniforms is another one, although that one is not penalty serving necessarily. The player with a non-compliant uniform is simply not allowed on the field. If he goes anyway, I suppose that would be a penalty every time he steps on.
