This topic will be used to keep teams updated regarding the Division B eligibility forms I have received. If you have any questions about the process, feel free to e-mail me or PM me. Remember, no hard copies need to be sent to me. It is only required that you fax the form to me. Your team should keep a copy of the original and the original should be sent to your conference. When faxing, make sure the team's main contact's phone number is included ON THE FORM. My fax number is 978-374-8895. You may fax at any hour you please.
Arkansas: certified between 1/21 and 1/28, faxed before 2/4
Augustana: certified between 2/24 and 3/3, faxed before 3/10
Cornell College: certified between 2/4 and 2/11, faxed before 2/18
Creighton: certified between 2/4 and 2/11, faxed before 2/18
Dordt: certified between 2/4 and 2/11, faxed before 2/18
Harding: certified between 1/28 and 2/4, faxed before 2/11
Memphis: certified between 2/11 and 2/18, faxed before 2/25
Missouri-Rolla: certified between 2/18 and 2/25, faxed before 3/4
Nebraska-Omaha: certified between 2/18 and 2/25, faxed before 3/4
Northwestern College: certified between 3/11 and 3/18, faxed before 3/25
Rose Hulman Inst. of Tech.: certified between 2/25 and 3/4, faxed before 3/11
South Dakota: certified between 3/4 and 3/11, faxed before 3/18
St. Louis Univ.: certified between 1/29 and 2/5, faxed before 2/12
Truman State: certified between 2/18 and 2/25, faxed before 3/4
Western Illinois: certified between 2/18 and 2/25, faxed before 3/4
Wheaton: certified between 2/11 and 2/18, faxed before 2/25

Sorry about that. You guys weren't on the composite list I received, but I have added you now. Here's what you need to do...
Log on to www.uslmdia.org/admin
If you don't know your username and password, have Jared Hedges (he is listed as the team admin person) contact me or your conference admin person (not sure who that is offhand)
Enter your complete roster
Print it out
Submit it to your registrar's office
Ask them to expedite it so that it is certified before the date I listed above
Fax a copy to me at 978-374-8895 (make sure someone's name and phone number is included)
Make a copy to keep
Send the original to your conference
