This topic will be used to keep teams updated regarding the Division B eligibility forms I have received. If you have any questions about the process, feel free to e-mail me or PM me. Remember, no hard copies need to be sent to me. It is only required that you fax the form to me. Your team should keep a copy of the original and the original should be sent to your conference. When faxing, make sure the team's main contact's phone number is included ON THE FORM. My fax number is 978-374-8895. You may fax at any hour you please.

Colorado School of Mines: certified between 2/4 and 2/11, faxed before 2/18
Colorado State - Pueblo: certified between 2/25 and 3/4, faxed before 3/11
Fort Lewis: certified between 2/2 and 2/9, faxed before 2/16
Montana State: certified between 2/11 and 2/18, faxed before 2/25
Northern Colorado: certified between 2/4 and 2/11, faxed before 2/18
Regis: certified between 2/4 and 2/11, faxed before 2/18
Utah Valley: certified between 1/21 and 1/28, faxed before 2/4
Western State: certified between 1/28 and 2/4, faxed before 2/11
Wyoming: certified between 3/10 and 3/17, faxed before 3/24
